The indications for advanced reproductive techniques have been extended to include almost every infertility problem: endometriosis, tubal factor, DES exposure, abnormal sperm counts, and sperm antibodies are some of the most common indications. Intracytoplasmic sperm injection (ICSI) is performed when the sperm count or motility is inadequate for IVF. ICSI can also be done with sperm removed directly from the testes.

In vitro fertilization was first developed as a treatment for infertility dueto blocked Fallopian tubes, but is now used to treat many different problems. To maximize chances of achieving a pregnancy, ovarian stimulation is used to produce many follicles (each containing an egg). During a normal, unstimulated cycle, only a single follicle develops and one egg is ovulated. Growth and development of follicles are monitored very carefully during a stimulated cycle. When the follicles (and eggs) are mature, a hormone known as human chorionic gonadotropin (hCG) is given and egg retrieval is scheduled 34-36 hours later. Egg retrieval is usually performed under light anesthesia and using ultrasound guidance, but under certain circumstances, may require laparoscopy.

Infertility is a disease of the reproductive process. It is defined as the lack of a pregnancy following one year of regular (at least twice weekly) unprotected (no contraception) intercourse and affects 10-15% of couple in the reproductive age group. For older women or those with known tubal damage, markedly irregular periods or in cases with a known male factor, waiting a year prior to consultation is not necessary and can be counterproductive. Remember, a womans fertility decreases with time and women over the age of 35 should be encouraged to seek consultation after going 6 months without conceiving.

Pioneering Fertility Treatment
To control the timing of egg ripening and to increase the chance of collecting a substantial number of eggs, fertility drugs selected for each woman's individual situation are prescribed. To monitor egg development, Genetics & IVF Institute utilizes ultrasound examinations of the ovaries (a painless method of imaging the enlarging follicles containing the eggs), and the measurement of serial blood hormone levels. An injection of the hormone HCG is then precisely timed to cause final ripening of the eggs.
